A fee is charged for requests for notification of purpose of use or disclosure. Please enclose the required amount of postal stamps when mailing the required documents.
For each request for notification of purpose of use or disclosure, 1,000 yen
No fee is required for other requests (correction, addition, deletion, suspension of use, erasure, or suspension of provision to third parties). -
◼︎Response to Requests for Disclosure, etc.
Responses will be sent in the method requested by the individual (or by the agent, in the case of an agent request).

◼︎Cases Where Requests for Disclosure, etc. Cannot Be Met
The following cases (①–⑥) may result in the inability to respond to requests for Disclosure, etc. If a response cannot be made, the Company will notify the requestor with the reasons attached. Please note that fees cannot be refunded.
① When identity cannot be verified
② When the authority of the agent cannot be confirmed
③ When the prescribed request documents are incomplete
④ When there is a risk of harm to the life, body, property, or other rights and interests of the individual or a third party
⑤ When there is a significant hindrance to the proper conduct of the Company’s business
